I have placed a Virtue: Testimonial Carousel on my custom side bar. I then installed MailChimp. Now in my Testimonial I have Chimp code

Where are the Testimonials stored?
Where do I find the Testimonial Carousel?
I found a Testimonial Form in the documentatnion( where can I get the form?

I may have inadvertently placed a [chimpy form] somewhere, and am truing to find it
