Home Icon Menu – Cannot Add 2nd Icon

I added the first icon just fine. It gives me the option to select an icon.
I was able to create a 2nd icon but do not remember how and have been unable to create a 3rd.
What I don’t see when trying to create the 3rd is the option to Select an Icon.
It does seem a bit inconsistent. I’m sure I’m missing the boat on something here but would appreciate your help.
