Contact form not functioning on mobile

The contact form (using the short code [contact-form-7 id=”108″ title=”Contact form 1″]) on mobile (iphone 7) shows only the data field Phone# available to enter information. The Name, Email, Subject, and Comment fields are inactive.
I don’t know if this this related to the graphics issue I’ve been having (in another post). In the process of trying to fix the other issue, the contact form was removed and had no effect on the graphic appearing or not.
