I used the SiteOrigin Contact Form widget to create a Contact page for my client’s website, maryaomalley.com.
In the Contact Page Options, I left what I recall is the default setting (Yes) for Use Contact Form. When I publish the page, two contact form now appear, side by side, the OOB form created from the SiteOrigin Contact Form widget (I surmise) and a second contact form created by retaining the Yes setting (I’m guessing, because when I change the setting to No and republish, the duplicate form no longer displays).
I created custom email addresses to use with the contact form (i.e., when the user types a message in the contact form that message is received in the inbox for that custom email address. In previous testing, messages sent from the contact form worked. Now they do not. I have submitted a ticket to DreamHost to request assistance for this issue. In case the issue is related to settings within the SiteOrigin Contact Form widget, I ask that you provide steps for resolving.

What settings should I use with the SiteOrigins Contact Form widget to display only one form and to make the published Contact Form functional (i.e., enable messages sent through the form to be delivered to my client’s webmail inbox on DreamHost)?
