Testimonial Input Form is producing lots of copys

Hi,
after i have got the latest update (maybe coincidence) the testimonial Input form is producing nearly 1300 entrys per one Input.
I have already made a new blind site from the originaly template, same result …
At the moment I have deacivated the input form ….
