How use the [kad_modal] short code

The documentation gives a very brief example of how to create a modal popup button here:

There is no real explanation of the following:
1. Where in the shortcode do we indicate the content that will be displayed in the modal pop-up window
2. How can we adjust the height/width of the modal pop-up window
In the end, my goal is display a video inside the modal pop-up window. The video would NOT be hosted on Vimeo or Youtube. I would love it if you can give an example of how to do this.
