Mysterious “Opt-in” form

OMG can someone please help me? For no know reasons, I now have this appearing at the very bottom of every page on my website:

It does not appear on my homepage or on any of my posts, just the pages.

I’ve tried disabling the email opt-in plugin I have installed, and tried a million other things but nothing seems to work.
