Contact Forms Not Displaying Properly on Pages

Hello,
I’ve created several contact forms for my website using the Virtue Premium Shortcode. These contact forms are now not displaying properly on my site. I’ve also noticed the Contact Form option is no longer available from my Virtue Premium Shortcode list. Here’s two examples of the forms not working correctly:
