Mailchimp integration stopped working

My integration to mailchimp was working but seems to have stopped. I’m not sure what I have changed to cause it, its been a while since I checked. I have a pop-up that invites people to subscribe, but now it doesn’t connect to mailchimp, I just get a wordpress confirmation that I have been subscribed. Can you have a look and tell me what I am doing wrong?
