Contact form not working

Hey guys,
My contact form isn’t working for the domain http://www.bydezignfurniture.co.nz
I’ve installed a check mail plugin, that’s working – confirming my wordpress installation can send email, it sends from the account associated with my wordpress login though. The form was working before, I’ve been onto my hosting provider and they haven’t helped.
Can anyone shed light on this?
