Display Testimonial Form on Front End

Hi Ben/Hanna,
I’ve looked but cannot seem to come up with how to display the testimonial form on the front end. Is it a shortcode? I’d like to put it on it’s own page and link to it from the testimonial grid.
Also, on the Pinnacle Premium demo, there is a second area on the testimonial grid page that shows a custom background and some nice transparent boxes for the testimonials. How do I invoke something like this?
