Accordion – table not showing

Hi
Referring to our test site http://www.haacko.co.nz
The two bottom headings – re Forms have the same Form opening in them.
Originally I had two different forms, the one on the right would not show when recaptcha was installed on the Form.
Now I have the same form on both with recaptcha installed.
a) The right hand Form A does not load
b) The left hand one does.
Any thoughts as to what I have done wrong?
Hope someone can advise?
