Country and State don't display on Checkout

I found a thread with a similar problem and tried the fix, but it didn’t work.

When I go to the checkout page the Country and State dropdowns appear initially, but as the page finishes loading they each disappear! A particular problem since they are both required fields. I’ve actually purchased and installed a WooThemes Add-on to not require this info, but that’s another matter.

It may be the way I have the “store” configured. My client wants folks (wholesale and retail) to be able to add items to their cart but will then contact them with a total and to arrange payment before shipping, so it’s an interrupted shopping cart process for lack of a better term. I had originally customized the COD option but thought that might be the problem and instead customized the text and message on the Cheque payment method. If there’s a better way to handle this I’m all ears! BTW, I also have a plugin installed to show wholesale prices from the same catalog IF the user logs in with a wholesale account.
