I have an html form that is automatically generated by a proprietary form in our office. You can see it here:

I would like to embed the full form itself (without the explanatory matter at the top) within a page on our site, so I can add material before and after, and so the page has the same overall look as the rest of the site. (I’m not concerned about the fonts; they can stay.)

I have tried to test the [embed] code in html, the [embed] option in the theme, and iframe code, and nothing will display the form itself. All I get is a link.

How can I display the actual working form within a page on our site?
