Images not sizing correctly and no captions

On this page I used page builder to add three images (in three columns) and I have two issues:
1) the captions are not showing, even thought I added captions
2) the images are not formatted correctly. I chose “crop images to equal height” and put “199” in the “set image height if cropping” Yet the images sizes (from left to right) are:
a) 800px × 640px (scaled to 293px × 235px)
b) 300px × 200px (scaled to 293px × 196px)
c) 300px × 240px (scaled to 293px × 235px)
What am I overlooking?
I am trying to accomplish something like this
