Plugin and Issue with Theme?

Hi there,
This is in reference to ticket #245313
I downloaded the plugin you had suggested, to create a more custom registration for WooCommerce. I have the form all setup but when I go to my theme options I get this error and can’t make any edits.

Any thoughts on what I can do?
