Portfolio: How to add Value 03 into the mouse over highlight effect

Hello guys,
could you please advise me how to add the Value 03 Title and Value 03 Description from my portfolio items into the mouse over effect of the portfolio thumbnails view (now there is only the name of the portfolio item).
