Shop inventory does not reduce when a paypal payment is made

Working on local host – AXS Fashions
Gone through PayPal Sandbox – OK.
Gone live and set the API’s – OK
Made transactions – everything OK, PayPal has the money in the sites PayPal Account of AXS Fashions
All Orders show “Pending” so nothing happens to inventory!
This is OK if you are a reseller and have 100’s on hand
BUT we are a Fashion House and only sell “One of a Kind”
We cannot afford to have customers argue over who paid first.
On receipt of a PayPal payment notifications we must reduce that item inventory to 0 -ZERO !!!
How is this done?
