SiteOrigin Contact Form Rendering Intermittently On Mobile

Hi there
We are currently using Pinnacle Premium and everything is currently up to date. We’ve been running into this issue for some time and cant come up with a solution. All mobile devices have been ios which are also up to date. Some of the fields in the SitOrigin contact forms seem to populate intermittently on mobile. Usually, refreshing the browser a couple times will correct the issue but we have tried it on nearly a dozen different devices (all ios) with the same issue. First link below shows one of the pages this is happening on, second link are a couple pictures of the issue. Any thoughts?
