ultimate member form issues

Hi
I am using ultimate member on the kadence theme. The registration fomr is doing 2 strange things that don’t appear when I switch the theme to twenty twenty.
1. the last field (Please let us know how you are involved with special education.), which is a drop down, shows up twice on most devices. (Not on an iphone though)
2. The privacy policy check box will not check.
I think this is a conflict with the Kadence theme. Do you have any advice?
